When an iphone is unlocked it means that it will work with any other gsm carrier like t-mobile. When you jailbreak that means that you customize you iphone and apps from the app store free with cydia.

To get technical jailbreaking is un encrypting the iphone filesystem so that we can add applications (Nowdays we just use an unencrypted RamDisk booted off the iPhone/iPod Touch with with iDemocracy 3 or ZiPhone) and unlocking is actually unlocking the Nor files, BootLoaders etc. of the iPhone so it can use any SIM card you want in it.

unlocking lets you use a phone on other carriers like tmobile or o2.
jailbreaking lets you install other third party apps and themes that apple doesn't let you download in the app store.

Very Easy. "Unlocking" is making the phone available to work with any carrier. "Jailbreaking" is making so you can install third party apps on your device.
